In order to take the survey customer will need to visit their local McDonald’s, purchase an item (i.e. Big Mac, French Fries) and then keep the receipt.  The receipt will have a 26 digit survey code which must be entered when taking the McDonald’s survey online at MCDVoice.com.  The Service Management Group will be charged with the task of gathering all feedback and distributing it to local McDonald’s managers.  Customers can sleep easy knowing their information will never be shared and that they will have a coupon for a FREE Quarter Pounder after taking the survey.  All feedback will be used to make the customers local McDonald’s a better place to eat and to increase the overall customer service of their employees.  Please note that the McDonald’s customer survey can still be taken without the 26 digit survey code but it is unknown if the customer will still receive the FREE Quarter Pounder coupon.  Customers are limited to 5 surveys per month per McDonald’s restaurant.

3. Enter the 26 digit survey number
4. Provide the store number, KS #, date of visit, time of visit, order number, and total amount spent (all of this information can be found on the customer’s receipt)
5. Answer a few short questions in regards to the McDonald’s visit/purchase
6. The MCDVoice coupon code will be shown
7. Write this code down on the customer receipt
8. Present it on a future visit to McDonald’s to obtain a FREE Quarter Pounder
